| What is a Medical Offer Report? |

| A complete analysis of the submitted claim file documents by one of our physician advisors. Upon completion of the analysis, a Medical Report will be provided that is supported by clinical evidence, not supposition, to be used to help drive the case to appropriate resolution. |

| Why would I need a Medical Offer Report? |
| |
| To assist the treating physician in a manner that will help to drive the case to appropriate resolution based upon the clinical evidence in relation to the issues of P&S, Disability Rating and Apportionment. According to the law, the medical report with supportive evidence carries the most weight. A peer to peer discussion between our physician advisor and the treating physician is most effective. However, if this cannot be done, a copy of the Medical Offer Report should be provided to the treating physician for his/her use in establishing a medical opinion based upon clinical evidence. The written opinion may also be submitted to an AME or QME physician. |

| When do I order a Medical Offer Report? |
| |
| This service is intended to be utilized prior to receipt of a P&S Report from the primary treating physician, and when: |

- MMI, according to ACOEM guidelines, should be or is being approached
- Permanent Impairment Rating will not accurately follow the AMA Guides or be
supported by clinical evidence
- Complex claims where there are opportunities for “other factors” not being
acknowledged or not likely to be appropriately apportioned by the treating physician
in the anticipated P&S Report
- You may also use a Medical Offer Report as part of the material to be presented to
an AME or QME in advance of their opinion

| How do I order a Medical Offer Report? |

One of our physician advisors will be assigned to the case the day we receive the referral from you. The process will be completed within 30 days.
Cost: Our fee is $1000 for standard, $1500 for complex or with Peer to Peer. |
